Job Description
We are seeking a highly motivated and compassionate Patient Care Coordinator to join our team at Cooper University Health Care. As a key member of our interdisciplinary care team, this individual will play a vital role in supporting patients and their families, serving as a patient advocate, and participating in telephone triage and scheduling patient appointments.

Responsibilities
This position requires excellent communication and teamwork skills, with a focus on fostering a culture of respect and collaboration among interdisciplinary care teams. The ideal candidate will be able to demonstrate evidence-based practice and nursing standards/procedures to promote excellent patient outcomes.

  • Main Functions
  • Provide education and support to patients and their families
  • Participate in interdisciplinary care teams to ensure seamless patient care
  • Assist in coordinating care and securing necessary prescriptions and supplies
  • Develop and implement strategies to promote excellent patient outcomes

Requirements
Minimum 1 year of clinical nursing experienceCurrent NJ-RN LicenseCurrent American Heart Association BLS Certification
