We are seeking a skilled and empathetic Professional in Elder Abuse Response to join our team. In this role, you will be responsible for providing expert support, developing tailored care plans, and collaborating with various stakeholders to ensure seamless care.

Key Responsibilities:

  • Develop and implement client plans, focusing on empowerment and independence.
  • Conduct risk assessments, identify safety concerns, and take proactive steps to mitigate harm.
  • Collaborate with healthcare providers, social services, and family members to coordinate comprehensive care.
  • Stay up-to-date with best practices, attending training sessions, and engaging in ongoing professional development.
  • Maintain accurate records, ensuring compliance with organizational policies and procedures.

Requirements:

  • Registered Professional (Social Worker, Nurse, etc.) or ability to obtain registration within a reasonable timeframe.
  • Recent experience in social services, preferably in the health sector, working with older adults.
  • Strong computer literacy, including proficiency in Microsoft 365 and CM Systems.
  • Clean criminal history.
