Rehabilitation Specialist

Job Description: As a Rehab Support Worker, you will be working closely with therapists to develop and implement Treatment Intervention (TI) plans. You will also link clients to community resources and support, utilizing diary management software to guide your work.
Main Responsibilities:
- Work collaboratively with therapists to execute TI plans
- Link clients to community resources and support
- Utilize diary management software and guidelines
- Communicate client queries to the leading therapist

Requirements:
- Maintain relevant board registration or APC where required
