**Key Responsibilities**

  • Medication administration and management
  • Conducting thorough patient assessments
  • Patient and family health teaching and education
  • Monitoring, assessing, and documenting changes in patient condition
  • Assisting with Activities of Daily Living (ADLs)
  • Collaborating with healthcare teams to develop and implement patient care plans

**Requirements**

  • Graduate of an accredited nursing program
  • Current registration with the College of Nurses and good standing
  • Familiarity with medical equipment, including monitors, IV pumps, ventilators, and feeding tubes
  • Previous experience in community nursing is an asset
  • Current CPR/First Aid Certification
  • Vulnerable Sector Screening within 3 months of hire
  • Excellent communication and problem-solving skills
  • Able to follow patient care plans and provide reliable transportation
