Job Description
We are seeking a highly skilled Needs Assessment Professional to join our team. As a Needs Assessor Service Coordinator, you will be responsible for completing holistic assessments of clients' needs and coordinating support services for older adults with health and disability related needs.

You will work closely with clients, whanau, and healthcare professionals to achieve positive outcomes. You will have excellent clinical reasoning skills, experience working in partnership with clients and their whanau, and strong communication skills. You will have a proven track record of positive relationship building and a commitment to delivering high-quality care.

Responsibilities
- Complete holistic assessments of clients' needs
- Coordinate support services for clients with health and disability related needs
- Work collaboratively with clients, whanau, and healthcare professionals to achieve positive outcomes

Requirements
- New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse
- Current New Zealand APC in appropriate scope
- Allied Health or Nursing qualification and relevant professional experience
- Full NZ Driver's License
