About the Role

We are seeking a skilled Clinical Needs Assessor to join our team at Te Whatu Ora Lakes.

The successful candidate will work as part of a multi-disciplinary team, assessing the needs of clients and coordinating support services to enable them to live safely at home or transition into Residential Care if needed.

Key responsibilities include conducting holistic assessments, developing and implementing care plans, and collaborating with clients, whanau, and healthcare professionals to ensure seamless service delivery.

This role requires strong clinical reasoning skills, experience working in partnership with clients and their families, and excellent communication skills.

Requirements
  • New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist, or Nurse.
  • Current APC in appropriate scope.
  • Allied Health or Nursing qualification and relevant professional experience.
  • Full NZ Driver's License.
