We are seeking a highly skilled and compassionate Needs Assessor Service Coordinator to join our team at Health New Zealand - Lakes. As a key member of our multidisciplinary team, you will be responsible for completing holistic assessments of clients' needs and coordinating support services to enable them to continue living safely at home or transitioning into residential care if needed.
This role requires strong assessment and clinical reasoning skills, as well as the ability to work in partnership with clients and their whānau to meet their support needs. You will also possess excellent communication skills and a record of building positive relationships with diverse stakeholders.
