The ideal candidate will have excellent clinical reasoning skills, experience working in partnership with clients and their whanau, and strong communication skills. You will have a proven track record of positive relationship building and a commitment to delivering high-quality care.

About the Role
- Complete holistic assessments of clients' needs
- Coordinate support services for clients with health and disability related needs
- Work collaboratively with clients, whanau, and healthcare professionals to achieve positive outcomes

Requirements
- New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse
