Child Healthcare Advocate

About the Position
We are seeking a Child Healthcare Advocate to join our team. In this role, you will work closely with our multidisciplinary team to coordinate care for children requiring life support home ventilation. Your responsibilities will include:
- Coordinating formal pre-assessment processes and supporting whānau entering the pathway throughout their healthcare journey to facilitate shared decision making
- Organising the discharge process, including MDT involvement; education for whānau, overseeing of equipment and liaison with the local Health NZ Districts to formulate a care package
- Working collaboratively and autonomously to coordinate the MDT and provide specialist advice with a strong patient and whānau focus
- Providing case management following discharge from hospital, including outreach to local Health NZ Districts to children and their whānau, and support of health professionals involved in any on-going care
- Coordinating multi-disciplinary and multiagency teams to ensure each child can live their maximum potential and in the best environment
