Job Description
We are seeking an experienced Social Worker to join our Community Mental Health team. As a Mental Health Clinician, you will work with individuals, their whanau, and the wider community to provide social work support and advocacy. Your responsibilities will include assessing needs, developing care plans, coordinating services, and connecting people with essential community resources.
Key Responsibilities
- Conduct ongoing psychosocial assessments to identify individual needs and develop effective care plans
- Provide social work support to individuals, their whanau, and the wider community
- Assess needs, develop care plans, coordinate services, and connect people with essential community resources
- Collaborate closely with a multidisciplinary team to monitor progress and provide crisis interventions when necessary
Required Skills and Qualifications
To be successful in this role, you will need:
- A Bachelor of Social Work or equivalent qualification
- Current Annual Practicing Certificate
- Minimum 1-2 years experience as a Mental Health Social Worker, preferably in a clinical setting
- Experience in mental health and addiction services
- A strong knowledge of community resources and networks within Counties Manukau
