As an Advanced Support Worker at Access Community Health, you will have the opportunity to work in a dynamic and supportive team environment, delivering high-quality care and support services to individuals in their own homes and communities.

Your key responsibilities will include conducting non-complex client evaluations, providing complex client care, and carrying out advanced tasks under the guidance of our clinical team. You will also take on a leadership role, offering training and mentorship to less experienced support workers, helping to develop and strengthen our team's skills and competencies.

Key Responsibilities:

  • Community-Based Assessments:
    • Evaluate support workers' care practices to ensure compliance with policies.
    • Provide in-field guidance to improve skills and enhance worker competencies.
    • Organize and lead workshops to enhance worker competencies.
    • Collaborate with care coordination and clinical teams to deliver seamless care services.
  • Advanced Client Care:
    • Deliver complex care and specialized tasks under the guidance of our clinical team.
  • Team Development:
    • Offer on-the-job training and mentorship to less experienced workers to help them grow and develop within our team.
