Our team of more than 600 believe that our elderly, those living with dementia and other conditions should be able to maximise their independence and engage, socialise and be active in their communities.This opportunity is Penrose based, with the ability to travel to the wider community in a company provided vehicle. This role has an 8-month fixed term duration, to cover maternity leave.As a Service Coordinator, you will ensure delivery of our client-driven and flexible community-based support services to the highest standards. This involves:Initial assessment and regular reassessment of clients in their own homeWorking collaboratively with clients, family and other agenciesDevelopment of individual support plans, in accordance with the clients' specific goal-based needsLeading, developing and managing a team of Support Workers to deliver the support and assistance to meet clients' goals and needsWe would love to hear from you if you have:A current Registered Nurse Practicing CertificateFull driving license with no restrictionsA proven track record in a health backgroundExperience leading a teamAbility to build relationships and networkAbility to work independently but also as part of a teamCapability to prioritise and manage your time effectively, with well-developed organisational skillsAn understanding of the restorative approach to service delivery in the home care-based environmentWe are Diverse and Inclusive:We are committed to providing a diverse and inclusive working environment.
