About the RoleWe have an exciting opportunity for an experienced Social Worker to join our inpatient service at Wellington Regional Hospital.
The social worker in this role will cover into all areas of the hospital when the usual social worker is on leave.
Areas include the Emergency Department, Medical, Surgical, Oncology, Renal, Cardiology, Orthopaedics, Neurology, Intensive Care Unit, Women's Health, Neo-Natal Unit and Paediatrics.
The role will be varied and interesting with the opportunity to flex into different areas of the hospital.
The Social Worker will be supported to upskill in areas they are not familiar with.
The role holder will complete psychosocial assessments with patients and their whanau and work in partnership with their allied, nursing and medical colleagues to facilitate supportive and safe discharges from hospital, with a focus on managing their health in the community.
They will respond to a range of referrals including addressing complex social issues, adjustment to new diagnosis, and support discharge planning.
The role participates in the Saturday roster.
The Social Worker will be allocated referrals by either the Social Work Team Leader or Social Work Coordinator.
About the Team / Service / LocationWe are a team of 21 social workers based at Wellington Hospital who provide a service to patients and their whanau across inpatient, outpatient and community settings.
We complete psychosocial assessments across the age range and provide holistic social work interventions.
This role is well supported by the other adult social workers in the team.

You will be able to work collaboratively and effectively as part of a multi-disciplinary team and with external agencies.
Working with a diverse group of patients with significant health needs the post holder will require excellent documentation, communication and time management skills.

Minimum RequirementsThe successful applicant will have the following skills and attributes:
NZ Registered Social Worker with current annual practicing certificateMinimum of 3 years social work practiceThe ability to complete psychosocial needs assessmentNavigating and working within the legal requirements of health care and social workA clear understanding of the health needs of Maori and Pacific Island communitiesTherapeutic social work counselling experience (desirable)Previous work in a health setting and a good understanding of health social work (desirable)Working at Health New ZealandHealth New Zealand | Te Whatu Ora is - the weaving of wellness.
