This is a permanent full or part-time role.
Kainga Tiaki Centre provides a multi-disciplinary, recovery-focused service for clients living in Central Auckland.
You will be part of a dedicated service, supporting excellent clinical care while developing your existing mental health knowledge and clinical skills in an established and multidisciplinary work environment.
As a referrals management clinician, you will have clinical responsibility for facilitating and managing tangata whai i te ora access to the range of Community Mental Health clinical services (CMHS).
This includes dealing with enquiries from the public and referrers, managing referrals, undertaking assessments, and providing general liaison between CMHS, GPs, other health services, and social agencies.
As a Key Worker in a care and recovery team, you will have clinical responsibility to utilize your knowledge and skills to assess all complex health needs of the tangata whai i te ora, providing care and advice that supports them to manage their overall healthcare needs.
You will support the tangata whai i te ora to coordinate their mental health needs.
You will provide active collaboration, planning, and liaison with the multidisciplinary team (MDT), general practitioner (GP), and any other healthcare providers, including NGOs and inpatient services, to enable them to perform their duties to their optimum.
You may practice in a variety of clinical contexts to provide a comprehensive range of therapeutic treatment options in partnership with the service user, their families/whanau, and communities.
We are seeking an experienced NZ Registered Nurse, Social Worker, or Occupational Therapist with a practicing certificate.
Our ideal candidate will have a high level of time management and organizational skills while demonstrating clinical skills in working with people from a range of cultural backgrounds in a mental health setting.
As this role is community-based, a full and clean NZ driver's license is essential.
