About Our Centre
\Kainga Tiaki Centre is a well-established community mental health team providing a multi-disciplinary, recovery-focused service for clients living in Central Auckland.
\We are seeking an experienced registered nurse, social worker or occupational therapist to join our innovative team in Mt Eden.
\This permanent full or part-time role offers the opportunity to develop your existing mental health knowledge and clinical skills in an established multidisciplinary work environment.
\As a referrals management clinician, you will have clinical responsibility for facilitating and managing tangata whai i te ora access to the range of Community Mental Health clinical services (CMHS).
\You will support the tangata whai i te ora to coordinate their mental health needs, providing care and advice that supports them to manage their overall healthcare needs.
\The care is based on comprehensive assessment, ensuring continuity, and is tangata whai i te ora-centred, culturally sensitive, and evidence-based.
\We are looking for a registered health professional with a breadth of experience in a Community Mental Health setting, who can provide active collaboration, planning, and liaison with the multidisciplinary team (MDT), general practitioner (GP), and any other healthcare providers.
