Through integration and collaboration, we're building a health system underpinned by partnership, equity, sustainability, whanau-centred care, and excellence.About The RoleTe Kaika Wellbeing Hub is a 'one stop shop' of integrated services established as a Treaty-based partnership between Te Kaika - Otakou Health, Te Manatu Whakahiato Ora - Ministry of Social Development and Health NZ.Tasks Include But Are Not Limited ToCoordinate the effective functioning of Te Kaika Wellbeing Hub and the Health New Zealand services.Ensuring administration and service standards are met and complaints are resolved.Ensure that Health NZ outpatient services delivered from Te Kaika Wellbeing Hub are run in a way which best supports the health needs of the community, enabling services to focus on reducing health inequities and disparities in access and outcomes.Establishing and developing relationships with key stakeholders and being the liaison for Health NZ for the day to day Southern operations at Te Kaika.Referral loading, wait listing, and booking of patients for outpatient clinics for Outpatient Services.Providing service support to the Charge Nurse Manager of 4th Floor and Frederick Street Outpatients and Ward 4b.Working with Te Kaika Wellbeing Hub partners to realise the vision of providing wraparound care that will particularly benefit Maori, Pasifika and low-income whanau while promoting rakatirataka through tikaka.Facilitating constructive communication and collaboration between partners and services and solving problems as they arise.About the Team/Service/ LocationHealth New Zealand - Te Whatu Ora - Southern has the largest geographical coverage of all the New Zealand regions, providing services to 350,000 people in diverse communities.
