You will work within a multidisciplinary team to deliver person-centred care in home and community settings.
You will work in a collaborative setting, sharing expertise and resources to deliver the best possible outcomes for our patients.
You will play a key role in assessing, planning, and delivering individualised treatment programs.
You will support a variety of outpatient cases, collaborating closely with multidisciplinary teams to provide holistic and effective support.
The working hours are between 7am-7pm Monday to Sunday.

About the Team/Service An Early Support Discharge Service is being established in the Nelson Tasman region, designed to help patients transition smoothly from hospital to home with the right support.
The team will work closely with patients, whanau, and healthcare providers to improve recovery outcomes and independence in the community.
We are seeking passionate professionals to join our dynamic team and make a real difference in people's lives.
About You Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growthExperience in projects/initiatives which give effect to Te Tiriti principles and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settingsA registered health professional with a minimum of 5 years clinical experienceExperience of working with other professions and MDT processesContract monitoring experienceExcellent interpersonal skills, a can-do attitude with a strong quality focusA capable leader but also a strong team member who is enthusiastic and innovativeWorking for Health New Zealand Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of Aotearoa/New Zealand.
