You will work within a multidisciplinary team to deliver person-centred care in home and community settings.You will work in a collaborative setting, sharing expertise and resources to deliver the best possible outcomes for our patients.
You will play a key role in assessing, planning, and delivering individualised treatment programs.
You will support a variety of outpatient cases, collaborating closely with multidisciplinary teams to provide holistic and effective support.The working hours are between 7am-7pm, Monday to Sunday.If you're passionate about helping people regain their independence and thrive at home, we want to hear from youAbout the Team/ServiceAn Early Support Discharge Service is being established in the Nelson Tasman region, designed to help patients transition smoothly from hospital to home with the right support.The team will work closely with patients, whanau, and healthcare providers to improve recovery outcomes and independence in the community.
