Right now, we - re looking for a
Registered Health Care Professional (HCP) to join the Community Services Team
and make a difference in outcomes for individuals who require care and support
to live in the community and maximise their independence. The service provides
support to a wide range of ages and care needs.

Day to day this role is focused
on undertaking client assessments and assisting them to set goals and identify services that will meet
their needs. There is an assessment
tool in place that works in conjunction with the experience and insight of the
registered health care professional and subsequent monitoring and evaluation of
their health status is also a key element.
This autonomous role works on a
rotational basis (with the other community team members) and includes triage,
hospital liaison and community.
The role is suited to a
registered HCP who can utilise their initiative and well-honed experience to
establish care plans to support clients and their families along with providing
client advocacy if required.
Success factors for this role includes
- Understanding of the Health of the Older Person Strategy

and other relevant national policy documents and legislation
- Commitment to enhancing the quality of life and
supporting independent community living and ability to manage client and family
expectations
- To be solution focused
- Able to work effectively independently and as
part of a multidisciplinary team
- Comfortable with technology
- Effective communication skills with a broad
range of people and professional teams
