As a Needs Assessor you will working
with individuals and families identifying and coordinating supports as required.
This role is office-based; however, much of the work takes place in the larger community.
You will be working with a wide variety of people, from the individuals
themselves, to providers, health practitioners, schools, and community
organisations.
Our aim is to support people, whose
lives are affected by a disability, to achieve their goals, retain their
independence and self-reliance, and to enjoy optimum quality of live wherever
they live.

You will be a health professional e.g. Social Worker, Occupational Therapist, Speech
Language Therapist or Registered Nurse. Registration is preferred but not essential.
Our
**team is looking for a person who has**:
- experience working with
individuals with disabilities
- experience
with assessment process and support planning
- the ability to work in both a
team environment as well as independently
- relevant post-graduate
qualifications (preferred)You
will enjoy a team environment and will be flexible and adaptable to service
needs.
You are required to provide proof that you have
completed your primary course of NZ approved Covid-19 vaccinations.
