Enliven Intensive Home Support, a service of Presbyterian Support East Coast (PSEC) provides support services for older people so they can live and enjoy life as a valued part of our community.
**In this community-based Social Work role, you will be supporting older people with complex health needs to maximise their independence and to live their lives fully in their community.**
**You will be working alongside and adding your value to a highly professional and experienced Multi-Disciplinary Team that strives for excellence and does so with a heart for the people we serve and each other.**
**You will be encouraged to bring a whanaungatanga and strengths-based lens to the Enliven team.**
**This role includes**:
- Case management and assessment
- Clear and concise documentation
- Liaison with clients, client whanau/family, other health professionals and services.
- Building and maintaining positive relationships both internally and externally.
**To be successful in this role you’ll be a qualified and Registered Social Worker with experience working within the health sector or with complex health and disability needs. You’ll also**:
- Have exceptional time management skills and excellent documentation and communication skills.
- Have highly developed clinical skills
- Be a team player and be reliable.
- Bring professional integrity
- Have energy and passion for the role
- Hold a full clean NZ drivers license.
