This organisation has been around since 2002 and works with more than 13,000 disabled people and their families across the wider Auckland region from Mercer to Wellsford.

As an Outreach Facilitator, you will be responsible for the planning and information process for Clients who are in public hospitals. Working collaboratively with hospital staff and clients, you will ensure they are receiving the right services and support to achieve desired outcomes.

**Responsibilities include**:

- Conduct detailed needs assessments to prioritise clients support needs
- Provide accurate information to hospital staff about eligibility criteria for disability services to ensure clients receive tailored services
- Communicate regularly with clients, whanau, hospital & rehab staff to ensure continuity with the provision and support of client’s needs
- Determine Support Packages, review, adjust and update as required
- Maintain accurate documentation in line with practices and processes
- Develop risk management plans for clients/families

**Essential to this role**:

- Health and/or disability related tertiary qualification
- Minimum 3 years work experience in health, allied health, education and/or disability sector
- Strong communication skills, particularly able to relate to a range of different people and diverse cultures
- Computer savvy and quick to learn new systems
- Confidence, maturity, emotional intelligence and an empathetic nature
- Proven experience providing support & services for people with a range of physical, intellectual and sensory needs

Having clinical knowledge and/or working in a hospital environment would be an advantage in this role
