**The details**: Two roles available, Part-time - 32 Hours per fortnight (0.4 FTE) - Rostered (Monday - Sunday, 4 hour shifts, 1630 - 2030 hrs.)

**Salary details**: $51,770 - $61,540

We require a dynamic health care assistant with a passion for rehabilitation in the community setting to join our team. These roles are working within a team of health professionals delivering rehabilitation to patients in the community. Although this position is based in Hamilton, our service covers the rural towns within the Te Whatu Ora, Waikato area. Travelling is therefore part of this position, and a vehicle is supplied.

START (Supported Transfer and Accelerated Rehabilitation Team) is a programme which provides and promote rehabilitation of patients within their home environment primarily for over 65 year olds and Stroke patients of all age. START aims to reduce the pressure on hospital beds, through avoiding hospital admissions, earlier discharge, preventing readmissions and improving the quality of care for this group of patients.

We require a dynamic health care assistant with a passion for rehabilitation in the community setting to join our team who has:

experience working in the community setting preferably with the older person

*
assist with rehabilitation activities within the patient's own home

*
work within an interdisciplinary team to meet patient goals

*
provide care delegated by a registered nurse or allied health professional

*
commit to ongoing education and development

*
be a team player

*
hold a full NZ drivers licence
