Community Coordinator

We are looking for a Community Coordinator to play a key role in helping to deliver on initiatives associated with the new Connect and Navigate (CaN) programme.
This is a fixed term 1.0 FTE role, working 80 hours per fortnight. Travel may be required across the Bay of Plenty region, so you will need to hold a full NZ driver's licence.
**He Kōrero mō te tūranga - About the Role**
You will liaise with hauora māori, community, primary care, and other health care providers to enable the engagement of those whanau who are identified as not engaged with health care services by building on connections made through COVID 19. This will include undertaking the patient navigator role when whanau do not identify with a Hauora or Iwi. The role is required to connect with and support community providers, sustaining staff to support patients, to gain the knowledge, skills with the tools, and confidence to reach desired health objectives.
This role primarily functions as a bridge, advocate, coach and mentor for the healthcare team, facilitating stronger relationships and active partnerships between, staff, patients, family/whanau, other relevant local community and government providers and organisations to address health wellness plans to achieve desired service delivery and health objectives.

- Understanding of general practice, long term conditions and common mental health concerns.
- Understanding of health literacy, cultural competence, whanau centered care.
- Knowledge of social services.
- Demonstrates, exudes and embeds behaviours consistent with He Pou Oranga Tāngata Whenua and the BoPDHB Manaakitanga values.
