Working as a Pathways Navigator you will be responsible for carrying out Pathways activities, to ensure people with needs, and their families, receive a professional and empathetic approach for prompt and effective access to services that meet their needs/goals.

**Responsibilities include**:

- Seek information proactively to establish eligibility and determine urgency and risk
- Advise eligibility for different services and refer accordingly
- Provide information, educate and advise contacts and clients about the eligibility, supports, services and access a wide range of providers and referrers
- Identify and prioritize client support needs whilst educating client on user driven process
- Make assessment bookings with clients and allocate initial assessment to the relevant staff member

**To be considered, you will have**:

- 2 years
- experience in a Clinical or Disability support position
- Health or disability related qualification
- Ability to communicate effectively in a timely manner that reflects sensitivity
- Excellent phone and administration skills
- Clinical experience preferred
