Located in Kawerau, our client Tūwharetoa ki Kawerau Hauora are seeking a trustworthy, confidential, well-organised Primary Health Practitioner to join their team. Established in 1995, our client provides quality health, social and education services for all who reside within the rohe of Tuwharetoa Ki Kawerau and Eastern Bay of Plenty.

**He aha te tūnga | About the role**
Providing support to 300 children and young people per annum, whilst managing approximately 15 - 20 caseloads at any one time, the key purpose of the Kaupapa Māori Primary Health Practitioner role is to provide a recovery-oriented community-based assessment, treatment and therapy service for mental health and addiction problems that is accessible, coordinated and effectively targeted for Māori. Facilitating group education programmes, with a priority focus on providing services and support for children and youth, the Service will be based within a Kaupapa Māori framework.

**Key responsibilities of the role include**:

- Facilitate group education programmes, with a priority to services and supports for children and youth
- Deliver quality Tikanga based education programs, including teachings, maurakau, korero porakau, mahi kai for schools and youth justice
- Assist in the development, implementation, and facilitation of new and relevant cultural services/programmes
- Promotion of language restoration
- Work alongside TKKH Kaimahi to support the development of culture-based work plans
- Work collaboratively with local stakeholders to promote cultural competency and awareness
- Create and/or maintain organisational cultural competency, safety and cultural sustainability.
- Participate at training sessions and meetings to ensure that information on culture, needs, concerns and aspirations of whanau are expressed
- Manage, monitor and coordinate care package requests
- Work collaboratively with team to ensure a holistic programme of care is maintained for wider whānau
- Completing reporting requirements, ensuring strict client confidentiality
- File management, ensure all documentation is accurate and managed as per protocol
- Participate in compliance audits and programme evaluations
- Registered Practitioner/Social Worker/Nurse or Allied Health Worker
- A minimum of five years’ New Zealand experience, working in a kaupapa Māori environment or organisation
- Knowledge and experience working with children, youth and families in a healing and reconciliation capacity
- Strong knowledge and awareness of tikanga Māori culture and history, particularly with mental health and addictions
- Knowledge and a good understanding of Te Reo Māori is preferred
- Strong relationship and network management, able to develop working relationships with whānau, internal and external stakeholders
- Excellent interpersonal and communication skills
- Counselling and interviewing skills
- Empathy, showing respect for clients and their whānau journey
- Excellent computer literacy skills, with proficiency in all Microsoft Office functions
- Excellent organisational and time management skills
- High attention to detail
- A commitment to safe work practices
- Positive, adaptable to change
- Willingness to undertake First Aid or other health and safety related training
