**The Opportunity**
Our client has an exciting opportunity for a Health Coach to join their medical team and be a part of an exciting new Whanaau Pai Programme. The medical centre has implemented a new model of service delivery that places health coaches within the facility as part of the practice team.
As the Health Coach, you will work alongside Health Improvement Practitioners, Support Workers, and the General Practice team to partner with patients experiencing issues that impact their health and wellbeing. In this role, you will support patients and identify their own priorities and gain the knowledge, skills, tools, and confidence to become active participants in their care so that they can reach their self-identified goals. You will work collaboratively to develop active partnerships with patients motivating them to make positive lifestyle changes, reduce high-risk behaviors, manage stress, and improve their health and wellbeing.

**The Medical Facility**
- Facility hours are on Monday to Thursday from 1.00 pm-5.00 pm
- Services offered: maternity care, diabetes management, chronic disease management, travel clinic, podiatry checks

**The Location**
This is a city in the Waikato region of New Zealand’s North Island. Hamilton Gardens, a sprawling public park, features elaborate themed gardens ranging from Italian Renaissance to Japanese and traditional Maori styles. The Waikato Museum displays Maori art and artefacts. Next door, ArtsPost gallery focuses on local art. In the busy city centre, Victoria Street is lined with restaurants, cafes and bars.

**Qualifications & Experience**
- Bachelors degree or equivalent in a health related field such as nutrition, public health, exercise physiology, psychology, health education, or similar.
- Previous experience working as a health coach or similar role.
- Certification as a health coach.
- Previous experience working in primary care directly with patients as part of the health care team.
- Previous experience working with Maori, Pasifika, and Youth.
