Clinical Team Leader Permanent 1.0FTE (80 hours/fortnight)
Early Discharge and Rehabilitation Service (EDARS)
Allied Health Team 3
North Shore and West Auckland
Health New Zealand - Waitematā
About the role
Are you a senior Allied Health professional looking for the next step? Or are you an Allied Health professional already in leadership or management and looking to make the move to the Waitematā District? If that is you and you are passionate about leading a team providing community based rehabilitation, we have a fantastic role available.
As the EDARS Clinical Team Leader, you will provide day-to-day leadership and management to an experienced and motivated inter-disciplinary team consisting of: physiotherapists, occupational therapists, speech language therapists, a social worker, dietitian, allied health assistants and a registered nurse. You will be supported in the role by the Allied Health Team Manager, other discipline specific Clinical Leaders and the Professional Clinical Leadership group.
This role includes ensuring quality inter-disciplinary service provision, supporting professional and workforce development and involvement in quality work.
As part of your role, you will also support referral co-ordination, which includes in-reaching to the inpatient wards at North Shore and Waitakere Hospitals to ensure that timely and suitable referrals to the service support hospital access and flow, as well as ensuring a smooth transition home for patients to begin the EDARS programme.
The role also includes a minimum of 0.2 FTE of clinical support, which could entail providing assessment and treatment to EDARS patients, teaching, training or completing audits.
About the Team/Service/ Location
EDARS focuses on providing a comprehensive, intensive and coordinated home-based rehabilitation service for people in the Waitematā community. The service delivers individually prescribed rehabilitation to patients over the age of 16, based on their specific needs and goals.
EDARS has two rehabilitation streams early supported discharge (more intensive rehabilitation where patients are seen within 3 days of discharge) and community based rehabilitation (less intensive rehabilitation where patients are ideally seen within one week of discharge).
EDARS provides a mixed model approach, to suit patient need and service demand, using in-person visits at home or in a clinic, or provision of rehabilitation via tele-rehabilitation where needed.
You will be joining an organisation that is forward thinking, innovative and values your contributions. The EDARS team use iPads or laptops to promote efficient community working and are involved in exploring new technology innovations to support clinicians in their work.
