We are Te Whatu Ora, Te Toka Tumai, Auckland Hospital. We provide health and disability services to more than half a million people living in central Auckland, regional services for Northland and greater Auckland, and specialist national services for the whole of New Zealand. Our main sites are Auckland City Hospital, Greenlane Clinical Centre and Starship Children’s Hospital, located in central Auckland.

**Te whiwhinga mahi | The Opportunity**

As a Community Nursing Triage Clinician, you will be responsible for timely and accurate community nursing triage and coordination of referrals to identified services, facilitating the appropriate pathway and workforce as part of the Adult Community Services single point of triage we are working towards.

**Nga Pūkenga Motuhake | Important Skills**

Your clinical decision making and communication skills along with proficient organisation and computer skills are imperative to be successful in this role.

Adult Community Services includes Needs Assessment and Service Coordination, Allied Health, Community Rehabilitation, Intermediate Care, Mobility Solutions, Specialist Nursing and District Nursing. There are approximately 230 clinical staff in community services that are focused on providing quality health outcomes.
