We're looking for a Kaitakawaenga to join our Long Term Conditions pathway. Our aim is to support pakeke and kaumātua with cultural safe services within the Long Term Conditions pathway at Health New Zealand - Te Whatu Ora, Te Matau a Māui.

As Kaitakawaenga, your role will be to initiate whanaungatanga and provide information Long Term Conditions services to Māori patients and whānau. You’ll maintain relationships with whānau while they are in our service, providing support to navigate Long Term Conditions services. You would work with whānau to enable them to take an active role in helping their loved one recover. You’ll work closely with the Team Leader and Clinical Co-ordinators to promote the Long Term Conditions Pathway in the community, and in turn will help to link our clients to community services.

You’ll bring your experience of working within the Health and Disability sector, and your exceptional interpersonal skills. You’ll be passionate about improving health outcomes, particularly for our Māori whaiora and their whānau and making a real difference to the people you work with.

This role is on a permanent basis, and there are part time or full time roles available.
