An exciting opportunity exists that requires clinical leadership within the ward environment and community health team by providing advanced clinical expertise and leadership in patient care. This role will lead and support the nursing, Medical and multidisciplinary team with the management of patient care whilst on their individual journey from admission through to discharge. The Nurse Practitioner will support patients who require complex discharge planning by working with the patients and whaanau, inpatient and community rehabilitation teams, and community support services.

We are looking for a Nurse Practitioner to join our Rehabilitation and Care ward at Pukekohe Hospital, Franklin Community hub who has the following attributes;
- A passion for older people’s health / chronic long-term conditions / Palliative Care
- Professionalism, kindness and commitment to quality care
- Proven leadership skills in a multidisciplinary team
- The ability to think outside the square, embrace challenges and pursue them with initiative
- Excellent communication skills, positive attitude and high level of confidence
- Motivated and enthusiastic
