Clinical Nurse Specialist - CRISS Cardiology / Heart Failure - Permanent part Time position Clinical Nurse specialist role working 0.6FTE - Professional and clinical leadership role - Excellent case and time management skills are key to this role.Mō te tūnga - About the role The Cardio Respiratory Integrated Specialist Services (CRISS) Cardiology/Heart Failure (HF) team is a dynamic, innovative and supportive team, ready to welcome a Clinical Nurse specialist with a special interest in Heart Failure. **Responsibilities**: within this role include the following (Also, refer to the Position Description). - Participating in Nurse-Led Heart Function clinics, working as a valued team member. The team includes Nurse Practitioner, Clinical Nurse Specialists and Kamahi Hauora Māori team members. - This team aims to optimise outcomes for Heart Failure Patients, in collaboration with the Cardiologists, Cardiology CNS's and Primary Care Ko wai koe - About You You will be an experienced Registered Nurse and be on the post graduate pathway, having completed or be working towards a relevant post graduate qualification. A comprehensive background in Cardiology is a prerequisite. As a positive role model in clinical leadership you will work with the CRISS Cardiology/HF team, providing advice, support and information throughout the patient's disease trajectory. You will work collaboratively with appropriate services, to ensure high standards of care are maintained and improved patient outcomes are provided.
