**Permanent, part-time 0.6FTE position**: - **Senior nursing position available to join the Diabetes Service**: - ** Diabetes nursing experience** **Mō te tūnga | About the role** The Specialist Diabetes Service is a multidisciplinary team that provides direct care for people with diabetes who have complex needs that cannot be met in general practice. Specialists will often have a role in coordinating population health strategies, facilitating education and training, leading and supporting new patient technologies, supporting primary and community care diabetes teams, research and surveillance of complications. We have an exciting opportunity for a senior nurse to join our team as a Clinical Nurse Specialist, taking on an advanced role and developing their scope and impact within the Diabetes setting. Currently working as a senior nurse, your passion and clinical expertise in this area will place you perfectly as a specialist resource for the team, providing valuable support and education to new and existing staff and nursing students. A confident communicator, you will easily establish quality relationships and ensure exceptional ongoing service delivery for our service users. **Ko wai koe **| About You** The Clinical Nurse Specialist will function in a collaborative model with the healthcare team to provide diabetes assessments, recommended treatments, patient and staff education, co-ordinated care and referrals to other service as indicated. This role will see you: - Providing leadership in clinical practice. - Acting as a resource for new and existing staff, and students. - Assist the Charge Nurse Manager to continue the nursing education focus in pursuit of clinical excellence. - Promoting nursing research and evidence based practice. - Contribute to processes, pathways, policies and procedures to ensure safe, quality care to people with diabetes in the outpatient setting. - Provide comprehensive care to patients using technologies to manage their diabetes and optimise their health outcomes. Your support and empathy accompanied with your excellent interpersonal skills and ability to build effective relationships with a strong patient focus will see you succeed in this exciting opportunity. **Ngā pūkenga me ngā wheako | Skills and experience** - Diabetes nursing experience - A postgraduate advanced health assessment qualification, with a view to obtaining a Nurse prescribing qualification is desirable - Registered with the Nursing Council of New Zealand with a current APC and mental health scope of practice.
