As a Tōtara Hospice Clinical Nurse Specialist (CNS), you will be providing advanced nursing care, focusing on complex patient as they journey through our hospice services. Working within our interdisciplinary team model you will utilise your advanced clinical expertise in assessment, care planning, and service delivery to meet the complex palliative care needs for patients and their families/ whānau.

Alongside working collaboratively with internal and external stakeholders to maintain strong interpersonal relationships, you will also support the further development of palliative care practices at Tōtara Hospice. You will role model and mentor nurses to ensure the effective coordination and delivery of our TH services.

Tōtara Hospice adopts a holistic assessment and care planning program that is supported by a Te Whare Tapa Whā framework where the team collaborates in partnership with the patient and whānau. This wrap-around care ensures patients live their last years of life well and enables you to find joy in your work.

The ability to work seamlessly following a patients journey within palliative care. You will have contemporary worked experience of care delivery models for both inpatient and community settings, and you can demonstrate your ability to build effective working relationships across all service streams and the MDT.

You would also need:

- Registration with the Nursing Council of New Zealand and hold a current APC
- Proven ability to coach and mentor in a clinical environment
- Sound knowledge of the Treaty of Waitangi, Tikanga Best Practice and demonstrated respect for diversity
- Experience in Palliative Care/ Oncology/ Chronic long-term health conditions
- Excellent verbal, written and presentation skills
- Excellent time management skills and be highly organised
- Postgraduate qualification/s
- PDRP at Expert/ Senior level
