We are part of Health New Zealand - Te Whatu Ora, the overarching organisation for New Zealand’s national health service. HNZ leads the day-to-day running of the health system, with functions delivered at local, district, regional and national levels.

**The Role**:
As a Clinical Nurse Specialist (CNS) for Gerontology, you will be an expert in clinical assessment and care for older adults across a variety of settings, including community, aged residential care, and Emergency Department at Health NZ Counties Manukau. You will be responsible for leading and providing expert clinical support to other health professionals, ensuring timely and appropriate care to optimize outcomes for older adults with complex health needs.

You will work collaboratively within the multi-disciplinary tea (MDT), supporting clinical staff and patients/families in providing patient-centred, evidence-based care. You will have proven clinical, education skills, coaching and mentoring ability, and strong interpersonal skills.

**Key Responsibilities**:

- Provide expert gerontology nursing knowledge, assessment, and care to older adults across the continuum of care, including community, aged residential care, and Emergency Department.
- Lead clinical care for complex patients, ensuring timely assessment and follow-up.
- Provide clinical support to Community Central, Virtual Clinics, and MDT meetings, working in partnership with other health professionals.
- Develop and implement care plans with a focus on person-centred care and evidence-based practice.
- Work in partnership with clinical teams to ensure the effective management of older adults with complex health needs.
- Involvement in the development of standards, policies, and protocols to improve clinical outcomes.
- Foster strong communication with patients, families, and healthcare teams to ensure a coordinated approach to care.
- Promote quality and safety initiatives and be involved in the evaluation of interventions and patient outcomes.

**Essential**:

- NZ Registered Nurse with a current Annual Practicing Certificate.
- 4-5 years of experience in nursing, specifically in gerontology or long-term conditions in NZ
- Postgraduate Diploma in Nursing (or equivalent) completed and working towards Masters in Nursing.
- Advanced clinical assessment skills in gerontology and care of older people.
- Experience in both hospital and community settings, particularly within aged residential care.
- Demonstrated leadership abilities, with experience in coaching and mentoring nursing staff.
