**About the role**
Specialty Clinical Nurses (SCN) working in the Patient at Risk Service (PARs) provide a nurse-led, hospital-wide service that ensures patients that are at risk of deteriorating are identified early, clinically assessed and undergo timely therapeutic interventions. It is a senior nursing role requiring specialist knowledge in identification, assessment, care-coordination and support with specific reference to patients at risk.
The PAR SCN is an integral member of the Medical Emergency Response Team and uses expert clinical knowledge to assess health needs and provide collaborative care with the multi-disciplinary team for deteriorating patients within Hutt Hospital. The SCN works both autonomously and in partnership with other health professionals, patients and their whānau to develop, implement and evaluate an integrated plan of care. The service is committed to providing support and education around recognising and responding to patient deterioration and is dedicated to continuous quality improvement to improve patient outcomes. This is a rostered rotating position with 12 hour shifts, covering 24/7. This is part-time FTE position. The salary range for this role is MECA Grade 1 Step 1 Senior Designated Nurse.
**About the Team/Service Location**
The Patient at Risk Service is a nurse-led team that provides critical care skills to the management of acutely unwell ward patients. Although based in the Intensive Care Unit (ICU), it has a hospital-wide responsibility and covers all areas including adults, paediatrics, maternity and outpatients. It provides a comprehensive 24/7 coverage for Hutt Hospital. The PAR Service supports patients, their whānau and healthcare professionals by: responding to all 777 clinical emergencies, acute referrals, referrals through Kōrero Mai, and monitoring patient - s recently discharged from ICU. The emphasis is on support and education to ward staff to help them recognise clinical deterioration and response in an appropriate escalation of care.
**About you**
We are looking for a skilled and enthusiastic registered nurse with an ability and passion to provide clinical expertise and leadership in the management of patients at risk of deterioration.
The SCN PAR Nurse will:
- Demonstrate excellent communication skills.
- Utilise advanced nursing knowledge and demonstrate expert clinical skills to aid in identification, assessment and care coordination of patients at risk.
- Have the ability to problem solve, work collaboratively and remain calm under pressure.
- Have a minimum of 4 years experience within the New Zealand acute care setting.
- Use advanced clinical skills with a priority given to those with existing competencies including NZRC Core Advanced, Paediatric Life Support, CVAD, cannulation/phlebotomy, NIV competence.
- Have a passion for ongoing learning and development with the ability to share, support and encourage others.
