- Permanent 1.00 FTE position (Monday - Friday with opportunity for some weekend shifts)

The Assertive Community Outreach Service (ACOS) is a multi-disciplinary, community-based outreach mental health service. ACOS provides assertive treatment and support to service users with psychotic and mood disorders who need proactive support to engage with services and/or treatment recommendations in order to promote recovery and reduce risks.

ACOS provides acute response and crisis resolution for service users during business hours, 7 days a week, with afterhours support from Te Toka Tumai's Urgent Response Service.

There are strong links with Regional Forensic Services, cultural and migrant services, accommodation providers and the homeless sector. ACOS provides proactive support into inpatient settings, therapeutic input for psychological issues, and a trauma-informed approach for 'high-risk' service users some experiencing homelessness, poor social determinants of health and contact with the justice system.

We are looking for an Advanced Practitioner (Nurse Specialist or Allied Health Advanced Practitioner) for this newly created senior forensic clinical role. Your responsibilities will include:

- Managing referrals and all transfers from the Mason Clinic Regional Psychiatry Service (Forensic Prison Team, Inpatient Units, Forensic Community Reintegration Service to Te Toka Tumai community mental health services.
- Maintaining collaborative working relationships with relevant parties and stakeholders, to facilitate and support timely and smooth transitions for whaiora from Mason Clinic Regional Psychiatry Services to Te Toka Tumai community mental health services.
- Attending monthly Forensic Prison Team release planning meetings and Mason Clinic Community Liaison Meetings
- Providing specific consultation and advice within Te Toka Tumai via participation in Forensic Case Review Meetings and High and Complex Needs Forums
- Providing clinical interventions and support to whaiora transitioning from Mason Clinic Regional Psychiatry Service to Te Toka Tumai community mental health services
- Managing a caseload within ACOS agreed with the Clinical Team Leader.

This is an opportunity for a registered mental health professional with excellent knowledge of forensic pathways to provide forensic clinical leadership to the specialist ACOS team that comprise a Clinical Team Leader, Clinical Coordinator, Mental Health Nurses, Occupational Therapists, Clinical Psychologist, Social Workers, Psychiatrist, Community Support Workers and a Team Administrator. The service is well supported by the Service Clinical Director and Operations Manager.
