**The Role**:
The Pasifika Health Service team is now seeking to appoint a Pasifika Health Coach / Navigator who is motivated to make a difference in the lives and wellbeing of the Pasifika population within the MidCentral region. We value understanding and taking the time to listen, so that we are able to effectively advocate for the care and services we provide our people, particularly those where language and understanding remain a barrier.
This role will see you working out in the community and at the office of THINK Hauora assisting with:
- Providing support to Pasifika families in all areas of health and well-being
- Work in partnership with the Pasifika Health team, Clinical Quality teams and Te Ara Rau Access and Choice within THINK Hauora and its external stakeholders to identify objectives relevant to this role that will contribute to the strengthening of Pasifika Health outcomes
- Support, guide and navigate persons with health needs and/or disabilities within the Pasifika population
- Active participation in team planning, activities and attending meetings
- Enter relevant Pasifika population reporting data on relevant PMS system
The role will suit someone who has worked within the Pasifika community, and is passionate about improving health outcomes for the Pasifika population. Ideally you will have:
- Experience working in either the primary or secondary healthcare sector
- Proven communication skills and a sound understanding of various Pasifika cultures residing within our district and the ability to communicate effectively with a wide range of people and services
- Fluent in at least one Pasifika language - preferably Samoan
- A current and full NZ Full Driver Licence
- Have an awareness and affiliation with the Pasifika population
- The ability to cope with change and be a driven self-starter
- A willingness to work as a team
