**About the role**

The role sits in the Intellectual Disability liaison team based within the community ID service. The role of the Care Manager is to act as the clinical manager for Care Recipients (under IDCC&R Act), to direct and oversee their care with appropriate consultation and input from other health workers while the care recipient is detained within hospital level services. Alongside the day to day aspects of overseeing the care of the care recipient, the care manager is required to develop robust care and rehabilitation plans and to ensure these are adhered to. They will liaise with identified people to ensure the care and rehabilitation plan remains a living document and its content evolves consistent with the needs of the care recipient and the recommendations of Specialist Assessors when reviewed every six months. This Care Manager role also includes contributing to specialist assessments and creating Care & Rehabilitation Plans for proposed care recipients in the community.

The care manager will also develop and maintain strong relationships with the Forensic Co-ordination Service (ID) and other Care Managers/ID services across the motu.

A significant component of this role will also be case management of RIDSAS clients (Care Recipients living in Regional ID Supported Accommodation Services). This requires a senior clinician to provide consult and liaison and specialised clinical input into the MDT team and NGOs for eligible consumers particularly around risk management.

**About the team**

The Intellectual Disability Liaison Team is part of the Intellectual disabled Persons Health Service (IDPH). The wider service provides services for adults across community and inpatient settings within their forensic and dual diagnosis teams.

You will be joining a multi-disciplinary team of health professionals committed to the delivery of gold standard care and rehabilitation to the population we serve. The team carry a wealth of knowledge and expertise in a number of areas relevant to intellectual disability, mental health, and behaviours that challenge across a range of disciplines.

We are a progressive service committed to providing a culture of ongoing professional development for staff through regular clinical supervision, excellent professional development and a recognition programme (PDRP) for nursing, regular in-service training and support towards external education.

**About You**

To be successful in this role these skills and experience would be expected/desirable:

- Experienced Registered Nurse with a current practising certificate
- Significant community and/or inpatient experience working with tangata whaiora with an Intellectual Disability and behaviours that challenge in either ID/Mental Health/ and/or Forensic services.
- Ability to work in a complex and demanding environment and being able to meet the needs of multiple stakeholders at the same time

- Current full drivers licence for both manual and automatic vehicles

The knowledge required by a Care Manager to manage the job appropriately can be seen as covering three overlapping areas.

These are:

- An understanding of the legal framework within which they operate.
- An understanding of the relevant legal processes.
- An understanding of key features of bio psycho-social variables related to Intellectual Disability and personality functioning.
- Excellent written and verbal communication skills
- Can work under pressure and maintain a balanced perspective

The appointee to the position of 'Care Manager' will need to have the knowledge, skills and attitude required to allow the Care Coordinators from the FCS(ID) service to designate them as the Care Manager under the terms of the Act for specified Care recipients and proposed care recipients.
