Join Our Team as a Social Worker, Disability and Older Persons
Are you passionate about making a difference in the lives of tāngata whaikaha Māori/disabled and older people? Ara Poutama Aotearoa is dedicated to the advancement of Māori health and equity, and we are seeking a dedicated Social Worker to join our Health Services Team. As a Social Worker, Disability and Older Persons, you will play a vital role in providing and promoting best practice in health and disability support services within a prison environment. This role may require inter-regional travel across our sites between Southern Auckland and the Far North.
About the Group or Team
The Health Services Team at Ara Poutama Aotearoa takes a holistic approach to service provision, encompassing general health, mental health, addictions, and disability services. This role is regionally based, you would form close working relationships with the prison-based health services teams, regional health leaders and the Lead Adviser Disability. Our goal is to address the health needs of the diverse population we serve, with a specific focus on improving Māori health and achieving equity of access and outcomes for tāngata whaikaha Māori/disabled people and older people. By adopting a multi-disciplinary approach, we ensure that our services meet the unique needs of the people in our care.
Ngā Kawenga | About the Role
As a Social Worker, Disability and Older Persons, you will be responsible for delivering exceptional care and support to older persons and tāngata whaikaha Māori/disabled people and their whānau within a social model of practice. Your role will include collaborating with a multi-disciplinary team, conducting assessments, providing interventions and evaluations, and making appropriate referrals to other agencies. You will engage with tāngata whaikaha Māori/disabled people, older persons and their whānau along with key agencies to access support and specialist services whilst the person is in prison and on their reintegration to the community. You will also be instrumental in the development, implementation, and evaluation of mātauranga Māori practices aligned with Hōkai Rangi.
Utu ā-tau | Salary
The salary range for the role is $60,606 - $70,173 (Mid) - $80,176. This is a permanent full-time position, and we may be able to offer flexible working arrangements. Appointment and remuneration will reflect skills and experience relevant to the role.
