Our client Tūwharetoa ki Kawerau Hauora are looking for a trustworthy, confidential, experienced Kaiārahi Whānau Ora Case Worker to join their team. Established in 1995, our client provides quality health, social and education services for all who reside within the rohe of Tuwharetoa Ki Kawerau and Eastern Bay of Plenty.

The Kaiārahi Whanau Ora Case Worker is responsible for working alongside whānau to develop plans and providing intense case management to support whānau in achieving their aspirations. Working collaboratively with all whānau members, providing advocacy and support in accessing services, and helping whānau learn new skills so they can transition from dependency to Tino Rangatiratanga are key duties of this role.

**Key responsibilities of the role include**:
- Undertake assessments and gathering of whānau information to identify needs
- Building rapport and working collaboratively with whānau to develop goals and strategies
- Develop and implement whānau aspirational plans to assist whanau
- Develop and implement programs/trainings or workshops that will enable whānau to achieve their aspirational goals
- Identifying needs and preparing referrals to external agencies for additional services
- Provide advocacy support when needed
- Promote a proactive approach to health and wellbeing and provide support to whānau to improve whānau outcomes
- Coordinate and facilitate activities, services, and opportunities to support the aspirations of whānau to become self-managing and take responsibility for their health, economic, cultural, social development, and wellbeing
- Completing reporting requirements, ensuring strict client confidentiality
- File management, capture date and update progress details
- Ensuring all documentation is accurate and are managed and filed as per protocol
- A minimum of three years’ experience, in a similar role performing similar duties
- Registered practitioner or social worker is desirable but not essential
- Strong relationship and network management, able to develop working relationships with whānau, internal, and external stakeholders
- Empathy, showing respect for clients and their whānau journey
- A commitment to safe work practices
- Experience working in a kaupapa Māori environment
- Knowledge of TKKH and the Eastern Bay of Plenty Communities
- Intermediate computer literacy skills, with proficiency in all Microsoft Office functions
- Positive, adaptable to change
- Willingness to undertake First Aid or other health and safety related training
- Knowledge of Tūwharetoa ki Kawerau tikanga and kawa
