**Te whiwhinga mahi | The Opportunity**

We are seeking a Transition Coordinator to join our team. The Transition Coordinator is a senior social work clinical role that partners with the Key Workers (Allied Health team) and the Oranga Tamariki liaison role based at Haumaru Ōrite.

Haumaru Ōrite is a supra-regional acute psychiatric child and adolescent mental health inpatient unit hosted by Auckland District Health Board. It is located on level one of Starship Children's Hospital.

Within the same facility Haumaru Ōrite also houses a specialist three-bedded Mother and Baby Unit which provides inpatient care to people living in Waitemata, Auckland, Counties Manukau and Northland.

This role requires a strong knowledge of statutory social work and will facilitate the co-ordination and development of multi-agency discharge planning for rangitahi.

- We are seeking a NZ registered Social Worker with at least five years mental health experience.
- It is also essential that you have a professional background in youth health and can work collaboratively within a multidisciplinary team.
- We expect you to understand bicultural issues and have a commitment to achieving equitable outcomes for Maori.
