**Kōrero mō te tūranga - About the role**
As a Clinical Nurse Specialist Wound Care you will lead the Specialist Wound Management Education and Liaison Service, a tier 3 service which compliments the Specialist Community Nursing Service Tier 2 Service (known as District Nursing) Nurses.
The focus of the service is to:
Oversee the management of referrals for the specialist wound management education and liaison service (a copy of the service guideline is available on request).
Provide expertise in the management of complex wounds.
Lead the designated nursing team that supports delivery of the Specialist Wound Management Education and Liaison Service.
Provide ongoing training of staff in evidence based wound care, doppler assessment and support staff in gaining skill acquisition.
Provide mentorship and support to Aged Residential Care and Primary Care nursing teams in the management of complex wounds.
Maintain nurse-led services/clinics in outpatient, home and community setting/s.
Strengthen the nurse-led case-management model for complex wound patients.
Collaborate and liaise with inpatient services to support best practice and facilitate discharge planning.
Contributes to data collection and reporting.
Initiate investigations (e.g. radiology requests) linked to own clinics or as requested by the specialist services virtual clinics e.g. request duplex scans.
Participate in the planning and delivery of education for health professionals that supports and enhances evidence best practice in wound management.
Leads and monitor the use of specialist wound products including trailing and auditing the use of products.
Provide advocacy for patients across the continuum for their health care related to their wound

The Community Health Service is made up of District Nurses, Associate Clinical Nurse Managers, Clinical Nurse Specialists, Health Care Assistants and Admin support and works collaboratively with a wide variety of services within the Community Adult and Allied directorate and wider Capital, Coast & Hutt Valley District Community.
The Specialist Wound Management Education and Liaison Service team is made up of CNS, Wound Care District Nurses and credentialed Doppler Nurses. The admin team support you by arranging clinic appointments, study days and other tasks as required.
Membership on the District wide Tissue Viability group role to support wound management standards, wound stewardship and education.
**Moū - About you**
We are looking for an enthusiastic, committed and experienced Registered Nurse with a passion for Wound Care and addressing inequities of health care in the community.
You will have extensive knowledge of wound care management, be confident completing a comprehensive nursing assessment and initiating clinical investigations to confirm underlying aetiology of ulceration, pressure injury and other non-healing wounds.
You will confident delivering education formally and informally with strong coaching and mentoring skills that support building wound care assessment and management skills.
You will confident working with a wide variety of clinicians, external agencies and service users.
You will be a member of the New Zealand Wound Care Society or other relevant organisation and have successfully completed an Expert Portfolio & Post Graduate Diploma in nursing working towards completion of your Masters in Nursing.
