40 hours per fortnight (0.5 FTE)
We have a vacancy available for a Clinical Pharmacist based as part of the renal team in Hawkes Bay. This role will be integrated within the renal team and support transition of care and appropriate prescribing of medicines in renal patients.
The Hawkes Bay clinical pharmacist facilitator team is the largest within New Zealand and has a great support structure with its own team manager, two team leaders and a clinical educator. This close-knit autonomous team of Clinical Pharmacists offers a supportive environment in a role that will allow you to reach your potential to work at top of scope.
Your main responsibility is to provide direct clinical pharmacist facilitation services, focusing on the appropriate prescribing and utilisation of medicines to the Hawkes Bay renal team.
Registered as a Pharmacist with the Pharmacy Council of New Zealand holding a valid Annual Practising Certificate (APC) with no restrictions.
Work experience sufficient to demonstrate general pharmaceutical knowledge in core areas and pharmaceutical knowledge in management of long-term conditions.
Current Driver Licence.
Great communication and interpersonal skills
Motivation to achieve exceptional outcomes alongside patients and the aged care team
Desire for lifelong learning and development as a pharmacist
Research, interpretation and provision of medicine information.
