**About the role -**

The Clinical Pharmacist (CP) in general practice is an expert in medicines and valued member of the healthcare team who ensures medicine use is safe and effective.

CP's support a model of care which increases the capability and capacity of the practice and where interdisciplinary team approach improves healthcare access and equity.
- Improve medicine optimisation
- Increase and optimise patient's adherence to their medication,
- Reduce medication confusion,
- Remove waste and;
- Improve patient self-management

**Do you have what we are looking for?**
- Pharmacy degree
- At least 2 years post graduate experience
- Proven experience in quality initiatives
- Experience with Patient management systems
- Excellent communication and interpersonal skills.
- Ability to build strong rapport with team members.
- Confident, friendly and approachable.
- You will ideally hold current CPR certificate
- Computer literate
