A new opportunity has arisen for two suitably qualified Clinical Nurse Specialists to join our team as we expand our Frailty Admission Avoidance pathway to better support older frail people at home following an acute episode or accident to avoid an ED presentation and hospital admission.

**Mō tātou Ratonga | About our Service**
Te Whatu Ora - Rehabilitation and Support, delivers a number of services to support people living in the community. These services work collaboratively with all other health care organisations and services.
The Frailty Admission Avoidance Clinical Nurse Specialist will work collaboratively with START, DSL, Primary Care, St Johns, after hour’s medical centres and emergence departments to triage and implement support immediately to prevent unnecessary ED presentations and hospital admissions. The service will become seven days a week, therefore will include a weekend roster.
