**Key responsibilities of the role**:
- Undertakes comprehensive assessments, risk assessments and formulates care plans.
- Manages a caseload and provide care coordination to tangata whaiora.
- Ensure whanau engagement and involvement are evident in all aspects of tangata whaiora care.
- Work collaboratively with other services and agencies to ensure outcomes for tangata whaiora are met.
- Demonstrate practice that reflects Health New Zealand | Te Whatu Ora’s values.

**Mōu | About you**
- Skills and experience you’ll need to be considered for the role:
- Knowledge and understanding of Te Tiriti o Waitangi.
- A commitment and passion to work with tangata whaiora and their whanau.
- Demonstrate a strong recovery focus.
- Effective communication and documentation skills.
- Hold a current full drivers licence.
- Hold a current annual practicing certificate to work in Mental Health.
- Experience or a passion for working in the Mental Health and addictions sector.
- Able to work with a multidisciplinary team and autonomously, providing proficient clinical assessments and intervention.
