Clinical Lead - Community & Rehabilitation (Northern/Midland)

**About the role | He korero whakamarama mo tenei turanga**
- Working in partnership with other Service Delivery leaders, you will ensure that the services we provide are clinically safe and responsive to client needs. You will be leading our Northern and Midland based teams of Registered Nurses with the support of two Senior Registered Nurses and two Clinical Nurse Specialists
- Contribute to the development and implementation of Service Delivery and Clinical strategies
- Provide clinical expertise related to contract requirements (including casemix), clinical practice, multidisciplinary working and complex client management to individuals and teams across the organisation
- Support the development of clinically sound models of care and translate these principles into service delivery processes and practices which both meet contractual requirements and support service innovation
- Strategically build relationships which support the integration and collaboration of service provision between HealthCare NZ and other primary and community care providers
- Mentor and coach clinical team members across regions and identify professional development requirements
- Lead the establishment and maintenance of robust clinical performance management systems and processes
- Contribute to ongoing Clinical Governance to ensure our clinical service delivery is contemporary and aligns with national strategic documents

**What you will bring | Nga tohungatanga mo tenei turanga**
- An active registration as a Registered Nurse and a current Annual Practising Certificate (APC)
- Relevant post-graduate qualification(s)
- Previous experience and in-depth knowledge of primary and community healthcare delivery models
- Demonstrated knowledge and experience in at least one of the following areas
- Long-term conditions and palliative care
- Rehabilitation and restorative care
- Neurocognitive impairment, including intellectual disabilities, dementia and brain injury
- Intensive support needs, including biopsychosocial needs which require multidisciplinary approach
