An exciting opportunity has arisen for a practitioner with minimum 5 years post registration and relevant clinical experience to provide coordination of the Post-Acute Community Rehabilitation Service.

The Post-Acute Community Rehabilitation Service (PACR) is a collective of services primarily dedicated to supporting clients immediately post their acute care episode. The PACR service aims to deliver the following initiatives consistently across the four localities (Palmerston North, Feilding, Tararua and Horowhenua) in the MidCentral region:

- Rehabilitation in the Home (ACC Non-Acute rehabilitation)
- Post-acute Community Stroke Rehabilitation
- Health Recovery bed therapy interventions to support slow stream rehab prior to returning home or receiving inpatient rehabilitation services.
- Transitional care beds to support Protection of Personal and Property Rights (PPP&R) process
- Voice therapy to support stroke rehabilitation, Parkinson disease and gender affirming intervention

The PACR Coordinator has a key role in the delivery of care, oversight and coordination of all clients across the separate initiatives and will support the profession specific Clinical Coordinators and Professional Leads in service development across these initiatives. This includes the provision of clinical and operational support of the members of the multidisciplinary team, to ensure visibility and consistency of service across the initiatives.

This is a permanent 1.0 FTE position

This position would suit either a Registered Nurse or Registered Allied Health Professional. Applicants will need to have a current annual practicing certificate. A current driver's licence is essential.
