**About the Role**

As a Health Care Assistant in the Discharge Lounge, you will work closely under the direction and delegation of the Registered Nurse to enable acute flow through the discharge lounge. Key focus for the role is:

- Working together to seamlessly transition patients out of acute care areas and onwards to their discharge destination
- Working in close collaboration with clinical areas and the Integrated Operations Centre to prioritise movements that achieve positive hospital wide system flow
- Exploring proactive initiatives to support the discharge process
- Ensuring patient and whānau discharge needs are met, including documentation, transportation, education, while bringing hospitality to the conclusion of their hospital episode
